Process

From phone call to first visit.

1. The call

Tell us what happened, who the patient is, and what the physician has said. We'll tell you immediately whether home health is the right service and what's needed to start.

2. Order & verification

We obtain the physician's order and verify coverage with the insurance plan directly. You are not the one making these calls.

3. Assessment & plan of care

A clinician completes an in-home assessment, and the plan of care is built with the patient, the family, and the ordering physician. Visits start on that schedule.

Common questions

Answers before you call.

Do we need a doctor's order first?

Yes — home health must be ordered by a physician. But you don't need to have it in hand before you call us. We regularly coordinate directly with physicians and hospital discharge planners to put the order in place.

What does "homebound" mean in practice?

It means leaving home requires a considerable and taxing effort, often with help from another person or a device such as a walker or wheelchair. It does not require being confined to the house. Medical appointments, religious services, adult day programs, and occasional outings are all compatible with homebound status.

How is this different from a home care aide?

Home health is skilled medical care — licensed nurses and therapists working under a physician's plan of care. Non-medical home care provides companionship and help with household tasks, and it is generally not covered by Medicare in the same way.

What will it cost us?

For patients who meet the coverage conditions, home health is typically covered by Medicare and by most other insurance plans. We verify your specific plan before any care begins, so you know what to expect up front.

Who actually comes into the home?

Licensed clinicians — registered nurses, physical therapists, occupational therapists, and speech-language pathologists, depending on what the plan of care calls for. You'll know who is coming and why.
